Housing Case Manager
Hours: Full-Time 40 hours (occasional weekend/evening events)
Reports to: Shelter Supervisor

Qualifications:
- Associate or bachelor’s degree in human services or a related field is preferred, but equivalent experience will also be considered.
- Basic knowledge, understanding and experience working with victims of domestic violence and/or sexual assault.
- Self-motivated, self-directed, organized, responsible.
- Ability to respond effectively to crises.
- Excellent oral, written, and electronic communication skills
- Skilled at building interpersonal relationships in the community
- Ability to maintain confidentiality.

Housing Responsibilities:
- Assess (ESG) Emergency Solutions Grant and (SDAP) Security Deposit Assistance Program client eligibility.
- Assist clients with housing applications.
- Work with clients to develop individualized housing plans, complete paperwork.
- Continually monitor and evaluate each client’s progression through their housing case management plan and develop corrective action provisions as needed.
- Prepare case management related reports.
- Maintain contact with grant administrator, property managers/landlords and other agencies, as needed.
- Complete housing inspections in accordance with the SD Housing requirements.
- Maintain client related data systems, including case notes and complete HMIS entries.
- Assist Executive Director and Business Manager with monthly ESG grant request or reports as needed.
- Provide referrals and information about other programs to victims.
- Work closely with the shelter staff to communicate needs and to ensure seamless continuity of services.
- Provide on-call crisis advocacy alternating weekend and holiday rotation with other on-call crisis advocates.
- Direct client services to include trauma informed care, advocacy, in-take, protection orders, and sexual assault.
